We provide specialist case management for children and young people with complex needs, developmental conditions or life‑changing injuries. Our case managers work closely with families, schools, therapy teams and local authorities to ensure every child receives coordinated, consistent and meaningful support. We focus on strengths, development and long‑term outcomes, helping each child reach their full potential.
Our Case Management Process
Holistic Assessment & Immediate Needs Assessment (INA): We begin with a comprehensive assessment of the individual’s physical, emotional, cognitive and social needs. If you are working with a solicitor, we create a detailed Immediate Needs Assessment, setting clear goals, recommended interventions and costed rehabilitation plans in line with the Rehabilitation Code.
Personalised Rehabilitation Planning: We develop a coordinated rehabilitation plan tailored to the client’s priorities, ranging from therapy, equipment and housing needs to education, employment or routines that restore independence and quality of life.
Multi‑Disciplinary Team (MDT) Coordination: We bring together physiotherapists, OTs, speech therapists, psychologists, nurses, support workers and statutory services to ensure seamless, joined‑up care. Communication is proactive, efficient and always centred around the client and family.
Rehabilitation & Therapy Access: We help clients access the right therapy at the right time, including physical, cognitive, behavioural and emotional rehabilitation. The aim is to maximise functional ability, well-being, and confidence.
